NEW STUDENT REGISTRATION FORM

I Agree as Follows;

• I will follow the instructions given to me by Flow Hot Yoga staff as to when, where, and how to perform Yoga. I understand that if I deviate from such instructions, it will be at my own risk.
• I will not hold Flow Hot Yoga staff responsible for any injuries suffered by me caused by any physical impairment of mine not fully disclosed to you in writing.
• I recognise that Flow Hot Yoga staff do all they can to keep my property safe, and will not hold Flow Hot Yoga staff responsible for any items lost or stolen.
• The tuition paid now and the fees paid in the future are non-refundable. Refunds, if any are made, shall be entirely within the discretion of Flow Hot Yoga.
